As a patient i have often used an exaggerated grimace to signal discomfort to my dentist.
i have heard that sometimes dentists will allow  a patient to sleep should he or she nod off in the 
middle of a procedure.
Perhaps sometimes a sleeping patient may need to be woken if the dentist requires a responsive 
patient.
In any event a dentist may need to assess the patients facial expression.
Current developments seem to be solely concerned with the oral cavity but should the area of 
interest be extended to the face as well.
