Issue 55: data request before sending order/inq

There will be one "account owner" at each partner who will be entitled to use to use the program and in charge for, we will fix it in a contract.
Therefore the name / phone number / email adress must be a fix data.
Our proposal is to put an 

"someone else" checkbox next to name field
"optional email adress" checkbox next to email adress field
"optional phone number" checkbox next to phone number field and if the box is chedked by the user, a box appears where he can fill other data.

Ok, but I would like to suggest, that we don't use checkboxes.
Just we put edit boxes as it is now, but they are filled by the default data at the beginning. The user will be able to overwrite those.
Maybe a "Reset to default" button can appear. If this button is clicked then the original "default" data is set in the edit boxes again

I have changed the specification of the excel (Specification_1.1) and added the name,email and phone columns on USERID sheet.

Ok, I have implemented the following:
The partner name, email and phone comes from excel. This is the default that appears in the send list dialog. It can be overwritten and can be restored by clicking on the "Restore to default" button.
If the default is chosen then an email is sent only to this email. Otherwise the emails's "to" is the default and the "cc" of the email is the email that has been given by the user instead of default email.
The details in the email will be the data that has been inserted by the user.
