What steps will reproduce the problem?

What is the expected output? What do you see instead?
I have tried various formats (MPEG-2 minimal reencode, Handbrake AppleTV, Quicktime H.264) 
and they all seem to cause some degree of audio and video sync issues.  MPEG-2 minimal 
reencode is definately the worst (off by several seconds).  I will occasionally get a show with 
Quicktime H.264 that has no sync issues, but it is inconsistent.  Am I the only one having this 
problem?  Is there a reliable format that works with Comskip and doesn't cause sync issues.

What version of the product are you using? On what operating system?
1.6 OS X 10.5.5

I don't know if you're the 'only one', but I'd suggest re-asking this on the forum:

Since people will read and comment (as opposed to here, which is mostly just read by me).

  The fact that the audio isn't in sync in the non-re-encoding formats isn't
surprising at all (it starts from an unsynchronized mpeg2-ts, so cutting that up
guarantees problems).  For the same reason, I expect anything using an external
encoder (like handbrake) to have similar issues.

  However, the quicktime setting should 'mostly' work.  (I use the hr-edl-seek flag,
which tells mencoder to encode the whole show, and adjust to remove those sections,
instead of just cutting them out immediately .. this is supposed to help deal with
such issues).

  I guess what I'm saying is any of the real mencoder encoding settings should work
somewhat well (with possible problems just around the cut locations).  If you're not
seeing that, definitely ask around, and if you figure out better arguments to pass to
mencoder I'll be happy to add them.  For what it's worth, I'm not noticing any a/v
sync problems on the shows I put on my iphone (using the iphone setting).
