What steps will reproduce the problem?
1.  Install database on MySQL with InnoDB table type.
2.  Insert a few changes.
3.  Restart the server.
4.  Insert some more changes, duplicate key errors should occur.


This is a "documented feature" of MySQL with the InnoDB
table types.  According to bugs 199[1] and 727[2], the
server resets the counter to MAX + 1 after a restart.
Which breaks the numbering scheme we use for these counters.
