The ssh_user_name column in the accounts table should have a unique constraint
to disallow adding multiple accounts with the same ssh user name.

ALTER TABLE accounts ADD CONSTRAINT accounts_ssh_user_name UNIQUE
(ssh_user_name);

actually, preferred_email should probably be unique, too.
