Affected Version: 2.8

What steps will reproduce the problem?
1. Push patch A to branch A
2. Push patch B to branch B using same ChangeId as 1
3. Try to change the topic on either Change

What is the expected output? What do you see instead?

The topic changes, but there is not enough information provided to the hook to determine which change caused the hook to fire.

Please provide any additional information below.

It is possible to reuse a ChangeId across branches and projects. The topic-changed hook doesn't provide enough information to resolve to a single change. It should either accept project and branch, or commit id.
