What Launcher app are you using? Google Now Launcher? What happens with other launchers?
ma...@marcardar.com <ma...@marcardar.com> #5
Just the default Launcher that is used by the Nexus 5. I haven't tried with any other launchers. But querying the AppWidgetManager gives the correct/desired information. So it seems to be a problem with the default launcher.

The problem is with the google launcher(latest version ). No issue with other launcher.

I'm also having this issue. I've struggled a lot while developing a widget, it doesn't show, doesn't update, reboots sometimes gets it working etc. All done with google now launcher installed.
Made default launcher default again, reinstalled the app, and the widget looks fine.
Android 5.0.1 on nexus 4.

this happens with nexus 6 too, it may be a google now launcher bug.

I am having this issue too, only happens on the Google Launcher that comes with the 5.1 image on my Nexus 5. The android:previewImage does not seem to load even after reboot. (The image file is definitely in drawable resources.)

I have the same issue with ALL widget I installed.
Note that the device (Nexus 5 32GB) has been "factory restored" after lollipop 5.0.1 OTA installation.
I tried to restore again, but the problem is still here.
After factory restoring also the "my library" widget is not functioning (listed into widget list but if selected it does not appear in homescreen). After reboot it can be correctly selected.

Hi, I can confirm this issue on two Nexus 5 (running Android 5.0.1 and 5.1) and a Moto G2 (running Android 5.0.2).
The issue should be also related to this one:
https://code.google.com/p/android/issues/detail?id=158485
The Android Project Member stated that this would be an Google Experience Launcher issue. I have already used the related "Help & Feedback" option, but I don't think that this will trigger anything.

Description
Related to:
Confirmed with Hanping Cantonese Dictionary app.
Reproducible steps:
1. Settings / Apps / Google App / Manage Space / CLEAR ALL DATA (or just install fresh firmware)
2. Install and run app (with homescreen widget) (e.g. Hanping Cantonese Dictionary). Maybe "run app" not necessary.
3. BUG1: From Homescreen, long press empty area / Widgets / <cannot find widget for app just installed>
4. Reboot device
5. From Homescreen, long press empty area / Widgets / <successfully locate and add widget>
6. Uninstall app
7. BUG2: Try to add the same widget again - observe that widget icon still exists - either with expected label and position in list, or new label equal to package name "com.company.package" (and so maybe new position). Attempting to add this widget gives the screenshots attached. Basically just errors which you would expect since the app has been uninstalled
8. Reinstall the app (afterwards, the widget can be added as normal) OR reboot the device (after boot complete, widget unavailable as expected, because uninstalled)
To recap, two bugs:
1. Widget not available after app install - need to reboot first
2. Widget not removed from list of widgets after app uninstall - need to either reboot, reinstall app or Clear Data on Google App app.
