I use trac as my issue management system, and in Trac, #XXXX is considered to be the ticket which has id XXX, whereas rXXX is considered to be the changeset which has revision number XXX.
By following this "convention", I can have direct links to revisions (instead of, today, tickets) in the "timeline" trac view.
I am agree that I am only talking about Trac, but I think others issue management systems also follow this "convention", as subversion itself names its revisions rXXX
