See my comment on  issue #524 .

Not sure whether it is appropriate to open a new issue for this, but I am
worried that no one will ever see my follow-up on that issue, and I can't
change the issue status.  What's the proper protocol?

Win7 don't install virtual printer which enables conversion on PDF documents, even if
the. Pdf files are associated with prog sumatra

There is one more issue with file associations that make it not work sometimes. I've 
identified the problem and will fix it.

Should be fixed by r1447. You can test with latest pre-release 
(http://blog.kowalczyk.info/software/sumatrapdf/prerelease.html) 1447 or later.
