I forgot to add that before doing this one instance should be pinned to the Dock. I mean, just launch MacVim, right mouse button -> Options -> Keep in Dock, then Cmd+Q. And then the repro. Output when none of those are running:

Could you try these steps?
Remove all MacVim icons from dock. Then launch MacVim by cmd+click-ing on iTerm. And select "Keep in Dock" on the launched MacVim icon on Dock. And launch MacVim by cmd+click-ing on iTerm again. |
This works great now, thank you. Opening a file either way uses the same icon. |
There are multiple icons in the Dock when one is pinned and the other one appeared by Cmd+click on a txt in a terminal. .desktop files have
StartupNotify=true
switch for such situations. There must be a similar one for .plist.macOS 10.12.3
