Though the gwt.js file is in fact Apache 2.0 licensed, the file does not 
itself contain the header.

Attached is a patch that will correct the header in the master gwt.js source.

According to 4.d. it should suffice to once state that your 'derivative work' is
under the AL, as far as I understand it that does not require you to reproduce the
terms on every file.
