non-optimal encodings:

we require user to specify OPSZ_0 to get c1: o/w we use Ib
we do document this in some INSTR_CREATE macros
we do not use 41 90 but instead 41 87 c0 since complex and we don't trust
that all processors treat 41 90 as a non-nop
