Some OPs require DH sessions to be used when forming associations even
though HTTPS is used.  DNOI currently ignores this suggestion and tries
once more to create an association of the suggested assoc_type, but again
using DH: no-encryption.

DNOI should respect an OP's desire to use DH associations in an effort to
interoperate with more OPs.
