What steps will reproduce the problem?
1. Just trying to use it
2. Login using OPEN ID
3. Authenticate

What is the expected output? What do you see instead?
I get a 404 error every time. I have never gotten this plug in to work

What version of the product are you using? On what operating system?
Version 3.3.1

Please provide any additional information below.
I need help getting this to work. PLEASE.

I too get this error.

However it only happens for the first blog on my multi-site WP 3.0.1 
installation. Other blogs created from this site work fine.

It's something to do with the way WP's .htaccess redirects everything back 
through index.php if you access the site root, or any subdirectory.

I have the same problem. My version is 3.3.3 and never got the plugin working.
