Works for me with KDE 4.4.5 on Mint. What icon theme is causing the problem? Does it happen with other custom themes?

I have been using oxygencolors(v3.0). I tried the new theme H2O-Icon-Theme and it works fine. It's possible that it's just that one theme, "oxygencolors." Thanks for the reply.

Ah, yes - it looks like the oxygencolors is broken - some of the icons are in the wrong folders, for example the "48x48/actions/" folder is full of 64x64 pngs. QIcon is at fault as well though, it shouldn't be returning icons that are too large.
Oh well... not my bug!
