Issue 155519: DefaultNotificationsSetting policy set to 3 (ask user) never allows notifications

If the DefaultNotificationsSetting policy is set to 3, the user is prompted to choose between allowing or denying permission when a website wishes to show notifications. This choice is stored as a user-set exception. However, user-set exceptions are ignored when the default value is set by policy (see issue 155516). The result is that the user is prompted to make a decision, the decision is recorded but never takes effect, effectively always denying the notification permission.

A further data point: The DefaultGeolocationSetting, which as exactly the same semantics and also stores the user's choices as user-set exceptions, does respect these when the policy value is 3.
