I fixed some bugs in acts_as_versioned.

I made the changes to the plugin itself and also extracted these as patches
into our vendor/plugins directory so we can easily re-apply them if we need
to update the plugin: 

  vendor/plugins/acts_as_versioned-fix_add_index_bug.patch

  vendor/plugins/acts_as_versioned-fix_automatic_create_bug.patch

These patches should be submitted back to acts_as_versioned maintainers.
